- Responsibilities
- Lifestyle Medicine program development & coordination
- Help support the growth and development of the practice’s lifestyle medicine services including patient education, one-on-one support, group visits, and wellness programs.
- Utilize a Bible-based lifestyle medicine framework to support physical, emotional, and spiritual wellness.
- Patient Education & Individual Support
- Coordinate the Chronic Care Management program & care plans for eligible members offering one-on-one support (Medicare CCM).
- Group Support
- Coordinate group lifestyle medicine visits with Physicians / Providers facilitating the communication with patients and helping to create a positive experience.
- Other clinical and administrative tasks based on experience may include:
- Registered Dietitian / Nutrition Counseling, Medical Assisting, Lifestyle Medicine Health Coaching, Community / Church Wellness Event Support.

- Skills
- Exhibit effective communication both orally and in writing.
- Possess organizational and time management skills.
- Ability to build strong, trusting relationships with participants.
- Ability to positively resolve participant challenges and objections about their health and behaviors.
- Ability to provide empathy in difficult interpersonal situations.
- Eagerness to learn new skills, including problem solving training, exercise training and motivational interviewing.
Located in California’s San Joaquin Valley, Hanford, home to about 60,000 has a rich history, culture, and community. “The Valley” is located between San Francisco and Los Angeles, with the Sierra Nevada mountains (Sequoia National Park & Yosemite) and central CA coast about 2 hour drives.
Hanford and surrounding towns are health professional shortage areas with diverse populations and medical needs. The need for full-spectrum primary care and the impact that quality professionals can give the community offers a sense of responsibility, gratitude, and fulfillment.
